This also could be related to your Aja board placement inside your Mac Pro. On the Aja support website, there is a section that shows which slots to use with the Aja board, depending on which MacPro you have.

I would go the route via FCP as well.

The technology for the OMF export was licensed from a 3rd party company. As far as I know they did not really upgrade their technology in the last couple of years, so the export is in the Media 100 application more or less only for legacy support.

I have the latest beta version of the next release installed on our Kona3 system, and with this software version Y/C output works as expected. Just tested with a 525i 4:3 29.97 timeline. I will try to downgrade to the current release version to see if it works differently, but unfortunately I still did not find the time to do so.

Render it from AE with a codec that supports Alpha channel; render it with RGB+Alpha, Alpha set to straight. If you have the ProRes codec installed, render as ProRes 4444; or render as “Animation” codec with alpha

If you want to bring in a still image, make sure that it has the correct frame size (720*576 or 720*486 for SD with the correct pixel aspect ratio for 4:3 or 16:9), or 1920*1080 or 1280+720 for HD. Best is to start with a AE video preset, since they contain the correct pixel aspect corrections and dimensions.
If you want to bring in an image, you can use many filetypes like PSD, PNG among others. If you want to bring in a movie, render it in e.g. ProRes codec, or DVCProHD or Animation codec or Media 100 HD codec among others to fast-import it into Media 100 (without an additional conversion). Again, make sure that you use a supported video frame size and framerate.

I will try to check it with our Kona3 this evening. Your timeline is SD4:3, PAL or NTSC? And you want to output a Y/C SD signal, right?

Sorry to hear that you are not happy about Media 100. I would agree that there are bugs in Media 100, but there are bugs in each application I´ve ever worked with. But overall Media 100 is much more stable and reliable for us than any other editing system, including FCP, Avid and others.
I haven´t heard about your bug where you lose audio files. Could you please describe what is going on there? In over 15 years using Media 100 we never encountered anything (software related), that caused a media file loss. Only drive problems gave us problems in this regard. -
