DVDATE, as Steven suggests, is the new and very welcome answer to this problem.
If you are editing in Premiere, then getting overlay timecode from AE is much less convenient than using DVDATE as a Premiere effect.

Perhaps it would be easier (and safer) to capture everything, make the changes, and then save the whole lot out to a new tape? Dodgy to rely on frame accurate inserts on a master tape even if you crack the technique.

I haven’t yet found a a need to alter my keyboard shortcuts, so I’m not well versed in the small print for PPro.
But I see that there is an option to use Avid XDV keys in the customisation window. Maybe that will help if you haven’t tried it already.
You may also get some more freedoms by hand editing the XML is the .kys files which you will find in the relevant sub-folder off the Premiere Pro program folder under Keyboard Shortcuts.

You might therefore want to try resizing the images to 1/4 screen in PhotoShop and see if that stops it.

As for options to re-assign, investigate the various elements of Edit Menu/Keyboard Customisation.

It is possible that your stills need deinterlacing if they were taken from video originally. Best do this outside Premiere. If my guess is correct, that will probably fix your problem. If not, come back for more help.
If you have not done so already, get the 6.01 upgrade (or 6.02 if you are on XP and have problems with 6.01). 6.01 fixes several significant issues with the basic 6.0 version.

It would be instructive to try capturing using Scenalyzer Live from http://www.scenalyzer.com. Free try before buy puts watermark on clips, but otherwise is fully functional. Get the v4 beta.
That way we can hopefully find out whether this is a Premiere issue or something to do with your computer.

No such problem here, even with two other effects as well as levels applied in PPro1.51. Both scrubbing and timeline replay show the combined effects in RT without rendering on my 3GHz standard FireWire-based system.
I’ve just had a play with all relevant replay settings, but none affected this behaviour.
So I have to guess that it an issue with your system – either with Premiere or perhaps with your graphics card/driver/DirectX or similar.

Playback of clips in the clip window uses a different rendering system (Windows) to that in the program window (Premiere), which is why some people report good clip replay and problem timeline replay when they have CODEC or other issues that are OK in Windows but a problem for Premiere.
Is it possible that you need to render the timeline? Invoking RT replay that challenges the hardware can cause the effects you describe, as can the active DV device issue already mentioned. Is there any chance that your source footage is not exactly matching the project settings?
