this may sound bizarre, but I do belive that all these various problems stem from one source, your video card. Try disabling opengl in your operating system and all adobe products. also, you may have conflicting older adobe drivers working with newer adobe drivers. I’d do a fresh install of Windows as a last resort.

    8-bit color (28 = 256 colors
    16-bit color (216 = 65,536 colors)

8 bit is bad, it loses data on blue colors. so render out at least 16 to reduce banding in nested comps.
you need at least 16 bpc to work in green screen for 64 levels of green or else you get a bad key.

its possible your colors are above chroma limits for NTSC, if that’s the case, it can create sharp lines, artifacts and line crawl. try a quick test with levels in-out 18-225 instead and compare the results.

first off, there’s no such thing as YUV lossless. it averages the rgb into overall brightness. second, interpret your incoming files so that they have correct color management into another correct project setting. Many files get saved with wrong color metatags in video imports. This goes for AE as well as quicktime players.

and also, moving from SDTV to HDTV color is lossy, it goes from YUV to YCbCr.

The main rule is: keep in same color space, color codec, and color bits.

extract effect or duplicated layer with levels effects and difference matte for alpha channel and a quick garbage matte will work wonders too.

if you’re HD broadcast, you’d be upper field and 1920×1080i pixels 16 or 32 bit, just in case…
    but don’t worry, most mid-level hdtv’s downconvert to a smaller 1366 x 768 from hd.
although AE’s built in broadcast color changer may leave some weird colors, so be careful and maybe try the broadcast tutorial that uses the levels function that just chops off the high and low ends of the brightness.

virtualdub with deshaker 2.2 for zooms, tilts etc. is free and just as good as monet.

another trick is smooth out the pan by sub sampling out the jerkyness. use twixtor to slow down 80% then take that comp and put it in a new comp with timestretch 80%, a right click. It will have exact same speed but way less noticable jerks in motion. It’s also a great way to turn 29p into 24p.
