You can use DL: replace by AE comp for each clip/still that needs CC.

But you can set sequence markers on the fly and the cut (ctrl+K) on the markers (ctrl left or rigth).

Mainconcept mpeg Pro HD 4 is your answer but unfortunately Smart rendering is not available under current Premiere Pro CS4 releases, because of a defect in its plug-in API. Future updates of Premiere Pro CS4 and MPEG Pro HD 4 will solve this.
It seems to work just fine under CS3

I have to ask the obvious: did you press ‘start queue’ in the Media Encoder. Do you see a yellow bar progressing?

If you change the scale to framesize in the preferences of a project to you need to re-import the footage otherwise the footage will stay at the original resolution.
