Make sure the wipes etc carrie an alpha channel like psd or png.
Jpeg does not have a alpha channel.

VOB from dvd recorders are known to cause issues.
If you have a camera with analog in connect the dvd player to the camera and the camera with firewire to the pc. This way you can capture dv-avi.

Try it the other way around.
Add Premiere Pro Sequence to the Media Encoder.
Or have a look at watch folders.

If you mean the Premiere folder in Program Files/Adobe, that is correct.
But that did not help resolve the issue, hmmmm

If its ac3 audio copy the ad2ac3dec.dll from the root of Encore and paste it in the root of Premiere.

As mpeg is a compressed file format the changes of getting out of sync with large file is not uncommen.
Chopping it up into subclips might do the trick.
