Import the files, let it build the cache and then import the XML. You’ll have two sets of files in your bin, delete the first set you imported before the XML and voila, you should be ready to go.
If you’re crashing on the audio conform/ cache building then make note of the file Premiere freezes on and see if you can pin down a corrupt file.

Creating a batch file with filenames seems less cumbersome and prone to errors than slipping and re-encoding in Premiere.
This is, of course, assuming that offset is consistent.
Instead of re-linking, you can also just use the group function; the latter will not show the offset warning. The downside is that both methods only create an altered file on the timeline, every time you pull a clip out of the bin you have to fix it. You cannot merge clips if the video and audio are completely from the same source.
I suggest the following: sync dailies to production audio with Plural Eyes, unlink and delete the dailies audio, fix the slight offset with the production audio and the clapper, merge daily video with production audio. It’s the least time consuming method that comes to mind besides resyncing and rendering an entire set of dailies. Anything less I feel would slow you down in ways you don’t anticipate.

Move around your edit and press T. T will allow for a mini-trim window and will work on the edit point on or nearest the playhead. Give that functionality a try.

Check your multiplexer settings. If it’s set to anything other than standard you’ll have issues. iPod multiplexing will remove b-frames at every level.

Strange, mind sharing your Premiere Pro encoder settings?
My previous results were CBR, low bitrate, I think 5Mbps. I tried CBR with an obscene 60Mbps as well as VBR with 60-90 Mbps as the range, and I was still getting the same M=4 and M=3 results.

AME uses MainConcepts’ h.264 encoder, as a point of reference, when selecting mp4 as the wrapper. It uses Quicktime when MOV is the wrapper. Just a bit of FYI so you know it’s not anything Adobe proprietary.

The human voice is rich in harmonics and overtones. Anything past 5-6K is considered “air” or “lightness” in terms of adding to the human voice.
To remove it before compression use a low pass filter effect. MP3 encoders usually incorporate a low pass filter, but they’re rarely as good as doing it yourself before compression.
