To record from the phone (Cell only) you could modify a set of earbuds by putting a connector in place of one of the ear pieces that will connect to your sound card input line in if you have it, and the other ear piece to listen with. You would need to turn the phone audio as low as you can and still hear it, since it’s at an amplified level, hotter than a line level and way hotter than mic level. If you have a audio mixer to gain stage the signal, that would be the best way.
As far as capturing video from your screen, you can use a free utility called Freecorder. It allows you to capture “Downloadable” audio and video from your computer screen. the captured video is in the .flv format. See this link.
