Probably designed in 3d, but you can fake something like it with displacement maps. These 2 tutorials will get you going:

Ah, thanks. Aside from doing it in AE, do you know what program is usually used for such effect?

Ehhh … that’s a little out of my expertise … you could do it with C4D … but I think Maya is usually top choice for 3D character animation. Getting your head around that program is no simple task though.
