Great question! Just letting you know… it happens to me too!
FWIW, we are running a few Symphonys on Unity. I though it might have something to do with different users/workstations digitizing clips from the same tape, but I don’t know. I know that it is not a repeated tape number in the same project because it will not let you add a new tape number if that number already exists in the project.
It’s a puzzle…
