I have just spent the last three days on the same dilemma as John Frey. I had just about given up when I read your post. I followed your clear and simple step by step, not thinking for a moment that this would work. It not only worked perfectly, but I get it!!!! I can now see the conflict that I couldn’t see before. Thank you so much for your solution, I am sure it will help others as this area of DVD menu creation can get a just a little confusing at first.
