Here’s the answer straight from Apple:

When performing reverse telecine to remove pull-down, avoid using large files wherever possible. In some cases, large files may result in issues with timing and audio sync. For the best results, use files that are under 9 GB for reverse telecine.

If you have larger files, you should split them or recapture them as smaller files before performing reverse telecine.

I just found something else: The Late-Breaking News about Cinema Tools 3.1.2 If you don’t have version 3.1.2, perhaps you should update:

Reverse Telecine File Size Limitation Resolved
An issue with performing reverse telecine on large files has been resolved. The only
limits to the size of the file that can be processed with the Reverse Telecine feature are
the amount of free space on the destination disk and any file system limits placed by
    the operating system.
