    I had the same problem (iMac, CS6) but it was an autorecovery file instead of the real file.

    Solution for me:
    If you right click on the file name on top of the document, choose Reveal in Finder. Then probably the autorecovery folder opens and you can remove the file from there. That’s what worked for me.
