I should add that this was specifically designed for the Mac version of MC. Most of the commands can be translated to Windows, though; for example, the command (⌘) key can be substituted with the control (⌃) key when using Windows.

Unfortunately my Photoshop skills don’t extend to creating text PDFs. I compiled the shortcuts in Numbers and exported that to a text PDF, but when I brought it into PS to make it pretty, it rasterized it.
