I think your problem may have to do more with the plasma/connection than BM. All the problems I’ve had were usually traced to HDMI, our SDI monitor always work. You could try getting an HDMI to DVI adapter and do some testing.
On the audio issue, keep in mind that you are connecting from a pro audio card, and the levels cannot be handled by consumer speakers or mixers. My advice on this is to go to your nearest audio store or online and order a couple of audio pads or a DI box.Here is one that will work. You could probably route the audio through system settings, but you will then get a lag due to the video being processed separately.

I noticed issues with 24P on Decklink studio cards when sending out to our client monitor via HDMI. The SDI out to the preview monitor work fine though.

The problem resides with Windows version of Quicktime. It’s also still 32 bits only, so I wouldn’t get my hopes up. VLC is a good alternative that doesn’t have the gamma issue and is much less bloated.

You may be able to copy the Premiere sequence from your AE project panel, and paste it back into the Premiere project panel, depending on how it was linked.

I just realized that you were using Snow Leopard. This is what I had to do in the past to apply permissions to multiple folders.

It’s quite possible that CS3 isn’t compatible with your current OS (especially if it’s Lion)
The other issue could be a video driver. I would check with the card manufacturer to see if there are any update for the particular card you are using.
Finally, it may not be compatibel with your current card. It’s been a few years since the last CS3 update.
