Your Blu-ray project settings should be set to the resolution of your menus, not your main content. If you are using 1920×1080 menus, then set it to 1920×1080, though I don’t recommend letting Encore do the upscaling, you should export a 1920×1080 file out of After Effects.
In the past we experienced errors during the build process with 24p menus, however you are welcome to try and see what happens. It is very easy to export a 29.97 file out of a 24p After Effects composition, as it can pull inter-frame data instead of just blending frames (except for pre-rendered footage of course).

Have you specified an SD resolution in the transcode settings for the individual videos?

Encore does not properly display the pulldown from a 24p m2v, so this is what happens. Your video should play back in sync once burned to disc. If you need to know where to place chapter markers, you should place them according to the audio, not the video.

I understand that, and I am talking MINIMUM. The target is the most important setting. For Standard definition I usually use a minimum of 1000 to 1500 kbps.

I’ve never purchased a movie through iTunes, so I don’t know for sure, but I would imagine it uses flash.

I’ve had issues in Encore when trying to use 24p menus for DVD or Blu-ray. Also, on commercial discs, I’ve noticed they use interlaced menus as well, even when the main content is clearly 24p. If your menus are 24p, you might want to try 29.97.

Make sure the interpret footage setting is correct for your 4×3 video files. So you say the 4×3 video looks smooshed… as in the opposite of stretched? Maybe you can take a screenshot?

Have you tried building a test disc (mount an image file, or burn to BD-RE) to confirm the final output is black?
Also, since you are creating a Blu-ray, you menus can be 1920×1080, even if you main content is 720p. So that is something else to try if you can’t get the 720p to work. Also, you menus shouldn’t be in 24p.

The Encore help documents will explain exactly how to create them using photoshop.
As for the dvd compliance, it uses sonic solutions code (creator of Scenarist) to verify it. It should be very compliant, however I am not positive as I have never personally reverse engineered the structure that Encore creates.
