Well, progressive frames do encode better than interlaced, however if you aren’t doing any downscaling (HD to SD), then the quality shouldn’t be too bad, especially at the bitrates you specified.
If you want to send me a sample segment of the DV AVI footage, I can encode it using a professional encoder (not Premiere), and you can compare the quality difference.

When you create a chapter index, the previous and next buttons don’t always carry over to the next menu (for example, the first menu will not have a previous, and the last menu will not have a next). When buttons don’t exist, Encore does not carry over the routing for them, and it messes things up. It is kind of a bug, as Adobe could implement a better algorithm for modifying the button routing when creating chapter index menus.

It could be the type of media you are using. Professionals use Taiyo Yuden (now owned by JVC), or Verbatim.

It cannot set an asset as first play, only a menu or timeline. You must place the intro animation into a timeline.

You could just create an animated motion menu for the entire background. In After Effects, extend the duration of the composition of your intro animation to the length you want your menu to display. Export the entire composition (or use dynamic link) and set as the motion background to the menu. Set the loop point of the menu to just after the intro animation ends. Let me know how that works out for you.

You need to create an image file of your project, then back that up. You can then use the free software ImgBurn to burn the image for duplicates (assuming you are PC, Mac users will need a different program).
