Chris, its nice that you collected so much on the one list.
I like to add turn of autosave aswell. Its ridiculous that having it turned on actually makes premiere much less responsive. -
I forgot to mention that I used Premieres own title tool aswell for som text. I know it sucks, but I wanted to skip jumping back and forth for once into AE.

Timelines settings:
I always work in the RED R3D 1080p 16×9 25p setting.Export Settings

YUV 10-bit and some times the RGBA 12bit when alpha channel is needed.
Always use maximum Render Quality. And never use previews.Formats in the timeline

Now I have uninstalled all adobe programs from the adobe manager.
Then I deleted them from windows programs and features.
Then I runned Adobes cleaner tool.
After that I have installed premiere (and AME wich follows along with premiere).Premiere is still trial and AME is working as aspected. I tried this twice with same result. Not sure what to try next.

I might have gotten it to work. For some reason the Mercury Transmit was turned of after the update and even thoe I have not activated the external output it looks I it did the trick. Need to use it a day or to to verify but looks like it fixed this problem.

The updates just showed up now. So atleast for me this problem has ben solved.

Tried to reboot creative cloud manager, install a new creative cloud manager. Reboot the pc and also to search for updates from inside of premiere.

I been working on this workstation for soon three years with no audio problems until premiere 2015.
The system is a two Xeon 2687w and 64g ram running windows 8.1
Its a supermicro chassi and motherboard. Nvidia gtx 690 for graphic.
Windows running of an intel ssd disk and media of an internal raid of 4 striped disks.I edit 5k and 4k (RED) if its no 3d of course (to long render times in with 3d in 4k).
But I tried with other formats while trouble shooting and its the same problem. -
If you opened a 2014 projekt or older you need to have the 2015.0.2 update installeld. Then you can hit your hotkey for the source settings panel. Its the old one so Its backward compatible.
I have some issue with the 2015 and 2015.0.2 thoe. Just have to give a little warning. But the source settings works fine with the 2015.0.2
