Unfortunately, that’s not how DVD players work, and it’s not possible author that kind of behavior.
After pressing the Stop button, pressing the Play button either resumes playback where it last was, or plays the first VTS written to the disc from the beginning.
The only way to get back to the menu when you’ve stopped the player is to press the Menu or Title/Top Menu button.

Yes, I think so – if you use the FABScript format for the subtitle, which is a combination of image files and text files. You’ll also need to decimate the image in Photoshop before you use it so that it’s a 2-bit graphic, which is all the highlight layer on DVD supports.
Sorry, I don’t have any further details on how to do this, and I haven’t done it myself. You’ll need to do some research.

Presumably you’re running Leopard (MacOS X 10.5.x) on this system?
Encore CS3 predates Leopard by a few months. You may want to consider updating to Encore CS4, which should be more compatible with Leopard, and has several Mac-specific bug fixes as well.

The error message is really only a warning – it’s telling you that the timeline end action won’t execute because there’s an end action on the last chapter which will execute first. If the timeline behaves as expected (check this in Preview mode, or even better, burn a DVD RW and test it in a DVD player), then you have nothing to worry about.
Then consider whether you need chapter end actions at all (and heaven forbid, any overrides). Playlists and chapter playlists are a much easier and cleaner way of doing things.

You could create a video background in Premiere that is the duration of your audio, with a still of the menu after your motion segment has played.

Not in Encore. You could add a watermark in Premiere for your video, and in Photoshop for any images.

Have you verified this on a burned DVD, or just in Encore Preview mode?
If only in Encore Preview mode so far, then try burning a DVD RW and testing it in a DVD player.

Unfortunately, you are stuck. 18 buttons is the limit for 16:9 menus (on DVD), but Encore’s Blu-ray menus have the same limitation.
You could work around it by creating two menus, and link one of the buttons on the first menu to the second menu, and have it auto-activate (and vice-versa with the second menu). This could be made to appear seamless to the end user, except if they view it on a software BD player, in which case they would need to click the button to get to the next or previous menu.
