I don´t have a solution but I also have problems with using the H4n as a voiceover microphone.
Premiere recognize the unit. in Preferences I choose H4 for sound In and internal speakers for Sound out. But when I klick OK it instantly switches the outsound back to H4. When I plug in my headphones to the H4n unit the outsound is very distorted and turning down the master in premiere doesn´t help so it´s not because it´s peaking.

I tried the same thing in Logic pro 9 and there was no problem.
So there´s something in Premiere that is causing this. Please help. Anyone?
