This is a bug in Encore that is triggered by Leopard. The issue does not exist in Tiger. Leopard was released after Encore CS3, so Apple did something in their new OS that exposed this flaw.
The only solution I know of is to downgrade to Tiger. Bad news for new Mac owners.

1. Encore’s transcoding engine is exactly the same as Premiere Pro’s. If there is a time-to-encode difference, then it is in the settings you have chosen.
2. Your workflow for converting NTSC to PAL is flawed. The proper tool is DVFilm’s Atlantis. At the very least, After Effects will do a better job than Premiere.
3. Did you use the same Encore project when you replaced the NTSC assets with the PAL versions? If so, what steps *exactly* did you follow to do the replacement?

Multiplexing doesn’t mess with the fields at all.
PAL video is normally upper field first, but PAL DV video is lower field first. If your source material is DV, then transcoding to UFF in Squeeze is what’s reversing your field order.

You can create a timecode counter in Premiere Pro and/or After Effects. You could also use those apps to create a progress bar.
If you have the Production Suite, then you can use Dynamic Link to set up a live link to an AE composition, which would allow you to easily make changes in AE and have them show up immediately in Encore, without having to render from AE first.
Use exported footage from PPro or the linked comp from AE as the video track for your audio timelines.

My experience is completely different; HF is grayed-out as soon as I link audio or video to the menu.
Create a brand-new project and experiment with a couple of menus and a couple of assets to see if you get the same behavior.
Another random thought – do you have highlight-only buttons in the menu? Using a Save As… copy of your project, try deleting the buttons temporarily and see if the behavior changes.
