I was just experiencing this same issue, and the fix, for me, was to change the preference setting for Audio Hardware (Preferences>AudioHardware) from “Built In” (which I assume is the default) to “Built In Digital Output” (my speakers are connected to the optical digital out).

This fixed the problem.
