    I have seen these web sites out there that allow you to input your name and e-mail address, and a short time later you receive a response containing a link to a video that has your name rendered into it numerous times.
    Can anyone tell me exactly how they are doing this?

    Most likely the video is embedded in a Flash swf file. The swf file is loading the text and placing it above the video layer. Flash allows interactivity, dynamic text, and can play videos (flvs at least and mp4s soon).

    Yep, that’s Flash.
    Learn the actionscript necessary to allow input into dynamic text fields.
    Then keyframe a bunch of dynamic text fields with preset paragraph and character parameters over an flv.
