Hi Henry,
Do you happen to have After Effects? If so, this is extreemly easy to do (motion tracking). If not, it’s not that hard to do in Premiere either – I just upgraded to CS3 from 6.5 (quite a leap) so I don’t want to mislead you so I’ll let others answer. If no-one does I’ll play around and make sure that I know what I’m talking about before I post..

I agree with Kevin, AE is the way to go for effects. In either app you would basically duplicate the track, apply your blur or whatever to the upper layer, than somehow mask the needed portion in the effect track.
In AE you can easily add a layer mask to the blur track. In Premiere you could use the Track Matte Key.
Here’s a tut I did for compositing and isolating in Premiere. It’s not exactly what you want to do, but it gives you the basic mechanics of how you could do it in Premiere…
