I have a 23.976 shot source video (on a 23.976 timeline) that I need to convert to 1080i/59.94 for delivery, and I’m racking my brain trying to come up with an end export that’s not choppy or out of sync. Is there a way to do this from within Premiere Pro CS4? Or outside of it, with a Windows machine?

I’d seriously, seriously appreciate any help.

search the help docs for 2:3 pulldown; you’ll need to add that to your footage on a 59.94I timeline. There is more info on the “telecine” page of Wikipedia as well.
