It’s likely some sort of corruption in the file itself. What was the source of the file? Is it DVCPROHD or?

What codec is the file you’re trying to import?

You sequence setups look like a disaster. You are controlling a 1080 50i source, capturing as AIC and then running the sequence as ProRes HQ 1280 x 720 30p!

Firstly none of your settings match – codecs and frame rates. If you are capturing 1080 50i HDV footage, then don’t convert to AIC. If you want to convert, make it ProRes 422 1080 50i . The sequence must then match this setting, otherwise you are transcoding to a different frame size and frame rate. This will make it play back badly and require constant rendering.

So apart from why one file won’t load, you need to sort what your sequence settings should be based on your source footage and also what your final delivery is.
