What is the best way to edit a Camtasia 6 screen capture in Premiere CS4 and then output to MPEG4 for delivery from a server? I can set up the project to match the AVI file exported from Camtasia. Are there any other ideas to keep the quality as high as possible?

Export your Camtasia project to full frames uncompressed. And make sure your frame rate is set to 30.

I would recommend using Quicktime pro to encode to H.264, exporting to Quicktime animation from Premiere. That will avoid gamma issues and create a slightly sharper image.
