Could sure be an install gone wonky… Not common, but not unheard of either. Pretty much it’s best practice to rebuild a startup disk from the ground up any time there’s an upgrade from one OS to another (aka Leopard to Snow Leopard) and any time there’s a new version of FCS (FCS 2 to FCS (2009). About the only time it’s not necessary is on a new machine installing Studio on it for the first time.
But doing it clean does really make a difference in my experience. We do this every 6 months at the 75 edit bay school I teach, and it’s paid off in extremely stable setups. I don’t think a single user setup needs to do this as often per se, but only when things happen as described above.
