30 Degree Rule
Does the 30 degree rule apply only in reference to horizontal positioning or does it also apply to the camera’s vertical positioning?

Horizontal only.
For most work where this applies (shooting a conversation around a table, etc.), unless it was done for specific effect you’d likely not want to change the vertical much at all… much less 30°.
