MP4 and m4v into FCP — not a new one !!

Ok before we get going yes I know about MPEGstreamclip very well – what I have is a really odd problem.
We are a Post House in London and do loads of Mood Tapes and Pitch reels which use elements from movies – NOT FOR RESELL.
So our old workflow was ; DVD play -> SDI ProRes onto XSAN -> Edit -> Archive = Nightmare on searching and project file sizes, Duplicate media etc etc etc
I started using Handbrake to create MP4 and m4v – could drag entire movie into FCP and BINGO BANGO all was brilliant !! as this was not going to broadcast the quailty rocked.
Step 2 was to intergrate Final Cut Server – enter metadata – Burnt in timecode so the producers could do soft Edits — worked a dream, I am now ready to retire …. but wait.
SOME of the MP4 and M4V will come into FCP some wont – same machine same settings no updates no idea ????
All of them open in Quicktime no worries and I can’t for the life of me see whats going on, issue is we have over 500 movie titles ingested no and the only way around it is to open in MPEGstream clip and export ProRes AGAIN !!!
Any Ideas ?
