Two odd issues in Premiere CS4…

A friend of mine is using my kit to check AVCHD footage for a short he’s directing. He’s had two odd issues arise, neither of which I’m sure I can be sure where they came from as I’ve not been around when they’ve arisen.
Firstly, he’s having bin appear to replicate when importing footage, i.e he’ll create and name a new bin, import clips into it and some time later the bin will be replicated within the main video bin. Does anyone have an idea what may be causing this and how to remedy it?
Secondly, he’s imported AVIs into an AVCHD project in Premiere and upon trying to view them in the preview panel in Prem, the timecodes don’t begin at 00:00:00:00, as you’d expect, but at a seemingly arbitrary place in the clip, even though the marker is at the beginning of the clips. Three of the clips appear to be timecoded concurrently, but oddly the clip shot last begins at 00:00:00:00.
These have already been logged, as much as it’s possible to thanks to circumstances beyond control, using a media player, so the question is, is it possible to reset the timecode for clips in Premiere so that the imported clips that you need to be all start at 00:00:00:00? Apologies for the vagueness of this post, but all the information I have is second hand.
