i always did them like this, as usually the clients saw the slate during screenings etc.

contact info/your nameof course you would edit the info per your needs. ie if its a show there is no agency etc.
almost all networks and some ad agencys have “deliverable requirements” which dictate slate info ie segemnt starts etc, (i remember doing spots for one soap maker, who required all this specific data on the slate AS WELL AS spoken info. it had to be a certain duration, at a certain location on the tape etc etc)
one thing for sure: the money man or woman gets top billing

My favorite requirement that I have received so far is this, and I am NOT kidding.
“A 20 second countdown shall precede the start of program and cut to black at 3”
“Master Tapes shall be configured as such:
09:58:00:00 – Black
09:59:00:00 – Bars and tone set to -18dB
09:59:30:00 – Black
09:59:50:00 – Slate and Countdown.”So I’ve been scratching my head for weeks trying to figure out how to squeeze that 20 second countdown into 10 seconds……..
