My transitions aren’t applying correctly. With video explanation!
I’ve set the default transition as dip to black. I select some clips and apply default transitions. Only some transitions get dip to black applied to them. I have to manually drag dip to black to the other transitions. When I manually do this the transition still works.

Another video. I have dip to black set at 2 seconds, center at cut. The first transition applied correctly. The second only applied for 1;16 (why?) and not centered at cut. I have to manually adjust this and it ends up working. The third transition is only 0;28 and start at cut (why???). When I manually charge it to 2 seconds center at cut it actually does become 2 seconds centered at cut. There’s nothing stopping the 2 second center at cut from working, but Premiere just doesn’t apply it.

You don’t have any “handles” on the clips. If a clip has a little triangle in the upper corner, that means it is the very beginning, or very end of the clip. When you TRIM some frames from the clip, the triangle goes away.
The old-school method of applying a transition meant that you would physically overlap the ends of two clips, for the duration that you wanted the transition to last. Maybe this will help you to visualize. Because the clips are butted together (which is how CS6 works), and you have a triangle, there is no material available to “overlap”.
If you have two “raw”, untrimmed clips on the timeline, butted together, and you want to add say a 1-second transition between them, then you should trim 15 frames from end of first clip, and 15 frames from beginning of second clip. Now you have “room” to place a 30 frame transition. In your video, I see the transition applied with diagonal lines running through it – this tells you there are no overlap frames. Trim the clip ends and it will work. Please review the HELP files for TRANSITION

I guess my issue is that for this particular transition there is no logical reason why there even needs to be handles, or overlaps. It’s dip to black. I can see you needing it for something like cross dissolve, but dip to black? And as you can see from the video, dip to black still works 100% if I just go in and force it to be 2 seconds, centered at cut. I just want the first clip to fade to black and the second clip to immediately fade into view from black. No overlap necessary. No extra footage necessary to create that overlap. In fact, there can even be a black gap of zero footage in between the clips and it shouldn’t affect it.
How do you select a bunch of clips and trim 15 frames from all of them in one go? I’m not clicking on 50 clips and manually setting clip points for each clip at 15 frames. Wouldn’t I need to trim 30 frames instead since my transition is 2 seconds long?

I would agree that it doesn’t seem like one should need handles for Dip to Black, but we don’t control how this stuff is programmed 😉
A few slightly more cumbersome work-arounds:
1) Put a Black Video clip above cut and add dissolves to both ends.
2) Same as above, but use Opacity sliders
3) Just use Opacity sliders on actual clips to fade down/fade up
And you are correct that a 2-second transition would need 30 frames on each side, I was just using the standard 1-second transition as an example.
