AJA io HD – video glitches, sound coming out of sync

Now I’m sure there have probably been similar threads about video glitches and sound coming out of sync before, but here goes:
We’ve been using an AJA io HD with a MacBook Pro running Final Cut Studio for about 9 months. After various teething problems – most of which were solved when our first AJA io HD went back with a dead component output, and AJA eventually provided the ‘DisableAltETTPolling’ plugin to enable us to actually capture video. However, our system has now started to drop or repeat frames when playing through a timeline, eventually resulting in audio being out of sync – sometimes by a few seconds.
The symptoms are as follows:
Play the timeline and it runs ok for a few seconds. Then the video will jump back a frame (sometimes more). Before you know it, the video has jumped several times, and the audio is completely out of sync. FCP does not report dropped frames.As a slight sidenote, if the playback is stopped, the AJA’s front display flickers for a moment.
The system has been working ‘fine’ for about six months, and we’ve edited several SD (PAL) projects on it, but at the moment it seems almost unusable as the user has to stop playback every couple of seconds in order to put audio back into sync. The problem is apparent via both component output (to our monitor), and by SDI to a DigiBeta deck.
Thinking about what’s changed in our system to cause this problem: we’ve installed XDCAM drivers to facilitate our new XDCAM HD 422 camera, but these can’t have affected the AJA’s output, can they? – especially when we reload projects which only a couple of weeks ago worked fine and now don’t.
I’ve updated the AJA software hoping this would fix it. Same problem.
Anyone got any ideas? Thanks in advance.
Equipment: MacBook Pro 17″ 2.6GHz, 250GB HD (7200RPM), 4GB RAM, Mac OS X 10.5.5, Final Cut Studio 2, Belkin ExpressCard Firewire 800 adapter, LaCie 2Big Triple 2TB Firewire 800 drive, AJA io HD (up to date firmware), various decks.

Did you make sure to update the IoHD firmware as part of the process? this sounds as if something is not fully functional.

Yeah, the firmware was updated, but the problem persists.

If you uninstall the XDCam software, does the problem go away?
