I think I am having a similar problem. Either the image freezes and audio keeps going, or I get a black clicker effect before the entire sequence goes black and audio keeps playing. I finally found a solution. The Hz of my headphones, mic and external speakers were all different. Even though they are not all technically working simultaneously, when doing voiceover all three are engaged and have to be the same.
